FIRST TIME VISIT

Thank you for choosing Dr. Ramirez and his staff to serve your periodontal and implant needs.

At the first appointment, Dr. Ramirez conducts a thorough review of  your medical and dental history and may ask questions related to the completed forms. The examination will follow beginning with obtaining your blood pressure and performing a thorough extra and intra-oral cancer screen. Next, the periodontal examination will be conducted with a special emphasis on any areas of concern by you or your general dentist. Necessary x-rays, scans,  impressions, and any additional diagnostic information will also be collected at this time.

Dr. Ramirez will spend time reviewing and discussing the findings from the examination and x-rays and will answer any related questions. A treatment plan consultation may also take place during this visit.

During this first visit, in order to allow us to provide you with the most optimal and efficient care, please be sure to bring the following:

• Completed new patient paper work (may be completed online under the Online Forms icon under patient info, or to the right/below)
• Any referral slips and x-rays from your referring dentist
• A list of current medications
• Information regarding any medical conditions which may require antibiotic pre-medication before your dental appointment (i.e.: joint replacements, serious heart conditions)
• Dental insurance, if you have it; please bring your insurance card and picture identification
